We appreciate your kind consideration of our office policies.  Their purpose is to provide consistent quality service to all our patients equally.  Of course, exceptions may have to be made under special circumstances.  Please feel free to discuss any such situation with us.

Appointments

All office visits are by appointment only. Please call the office at (302) 628-4370 at least 24 hours in advance to cancel or reschedule an appointment. In fairness to all our patients we cannot serve walk-ins.

Please let us know of any changes in your address, phone number(s), or any other information.

Please present your current insurance card(s) at every appointment. If you do not have your current insurance card(s) with you, your appointment may be rescheduled.

Please DO NOT bring food, drinks, or candy into the office. Smoking in the office is prohibited by law.

Please DO NOT bring children to your appointment. We love children, but because of limited space in the waiting area, consideration for other patients, concern for the child's safety, and potential distraction during the visit your appointment may be rescheduled.

Insurance

Please let us know of any changes in your primary or secondary insurance, policy numbers,
or co-pay amount

All insurance co-pays are due at the time of visit.

Know your insurance coverage. Find out if your insurance requires: a specific network of doctors; designated laboratory or radiology facilities; referral forms; authorizations for special tests, consultations,
or Emergency Room visits. You should contact the toll-free Customer Service number, usually located on
the back of your insurance card, if you have any questions or concerns regarding your coverage.

Please request all your referrals and prior authorizations at least 72 hours in advance.

Forms

Usual form completion fee is $5.00 per page. Fee is required prior to completion of the form(s). Fee may be waived under certain extenuating circumstances (for a specific reason only). Forms are processed on "first come - first served" basis, depending on the amount of office work pending at the time. No specific time can be given for the completion of form(s).
Expedited form completion fee is $10 per page. Form(s) will be reviewed and completed within 72 hours after receipt of processing fee.

Prescriptions

Please request your prescription refills at least 24 hours in advance. You should never wait until you run out of your medication.

Please DO NOT ask the receptionist for your medication refills. In order to reduce potentially dangerous medication errors, we request that all your medication refills are handled by physicians, pharmacists, or registered nurses only.
